For admission to The Medical Center at Franklin, it is helpful to bring certain documents and information. This may include:

• Personal Identification (driver's license, birth certificate, etc.)
• Insurance card(s)
• Medicare card
• List of current medications (prescription and over-the-counter)
• List of allergies
• Copy of living will or advanced directive
• Contact information for local closest relative

Personal items to bring:

• Robe
• Slippers
• Comfortable clothes and shoes
• Book or magazine
• Hairbrush, comb, shampoo, toothbrush and toothpaste
• Items such as contact lenses, eyeglasses, hearing aids, dentures, and prostheses

About medications:

The hospital will provide all the medications you need during your stay. You still need to bring a list of medications you normally take (prescription and over-the-counter).

Please don't bring:

• Jewelry
• Large amounts of cash
• Other valuables
